Welcome to Mind Crew!

School

Introduction

Thank you for taking the time to complete this questionnaire as part of the Mind Crew programme.

Mind Crew is a structured 12 week emotional well-being programme designed to help primary school children develop emotional awareness, resilience, positive relationships, confidence and self-worth.

As a teacher, your observations provide an invaluable perspective on how a child manages their emotions, responds to challenges, interacts with others and approaches everyday school life.

This questionnaire will be completed before and after the programme to help evaluate any changes in the child's emotional well-being and the effectiveness of the Mind Crew programme. We therefore kindly ask that the you complete both the Initial and Final questionnaires.

Please consider the child's behaviour and emotional responses over recent weeks, rather than focusing on isolated incidents.

There are no right or wrong answers. We recognise that children may behave differently across environments and that some skills may not always be observable in school.

For each statement please select one of the following:

  • Strongly agree

  • Agree

  • Not sure

  • Disagree

  • Strongly Disagree

  • Don't know/Cannot Say

Please answer as honestly and objectively as possible, based on your observations.

Thank you for supporting Mind Crew and helping young minds navigate life.
