Teacher Questionnaire

School
Are you the same Teacher who completed the Initial Questionnaire for this child? * Please fill in the Questionnaire if you answered "Yes".
Yes
No

Thank you for supporting the child throughout their 12 week journey.

As the programme comes to an end, we would appreciate your observations of the child's emotional well-being, confidence, resilience and relationships within the school environment.

This questionnaire contains the same statements as the initial questionnaire and will help us evaluate any changes observed during the programme.

Please base your answers on the child's behaviour and emotional responses over recent weeks, rather than on any isolated incidents or how they presented before joining Mind Crew.

There are no right or wrong answers. We recognise that children develop at different rates and that some changes may be more noticeable than others.

Please answer as honestly and objectively as possible, using the same six response options.

We also kindly ask that you only complete the questionnaire if you completed the initial questionnaire for this child.

Your feedback is invaluable in helping us understand the effectiveness of Mind Crew and to continue developing the programme to support children's emotional well-being.

Thank you for Helping Young Minds Navigate Life.

Mind Crew - Teacher Questionnaire

Privacy & Confidentiality Notice

At Mind Crew we are committed to protecting the privacy and confidentiality of children, families and school staff participating in the programme.

The information provided in this questionnaire will be used to understand children's emotional well-being within the school environment and evaluate the effectiveness of the 12 week Mind Crew programme.

Mind Crew will collect and analyse questionnaire responses to measure changes in children's emotional awareness, resilience, relationships and self-worth.

Participating schools will have access to summarised results and progress graphs relating to their own pupils through the secure Mind Crew schools Hub.

Each child will be identified using their unique Mind Crew reference number rather than their name. Questionnaire responses will be securely stored, with access ro authorised individuals involved in programme administration and evaluation.

Results shared more widely will be anonymised or aggregated, with appropriate safeguards to prevent individual children from being identified.

Personal information will be handled in accordance with applicable UK data protection legislation, including the UK GDPR and Data Protection Act 2018.
