Welcome to Mind Crew!

School

To protect your child's identity, we ask you to use their unique Mind Crew reference number rather than their name. This allows us to compare responses before and after the programme without identifying individual children in our evaluation reports.

At Mind Crew, we believe that every child deserves to feel confident, valued, understood and equipped with the skills to navigate life's challenges.

This questionnaire helps us understand how your child is currently managing their emotions, confidence, friendships and everyday challenges.

As a parent or carer, your observations are incredibly valuable. Your answers will help us build a picture of your child's emotional well-being and identify areas where they may benefit from additional support.

The questionnaire will be completed at the beginning and end of the 12 week Mind Crew Programme, allowing us to evaluate any changes in your child's emotional well-being, confidence and resilience. Therefore we would very much appreciate it if the same parent or carer could complete both the initial and final questionnaires.

There are no right or wrong answers. Please respond based on how you feel your child has been managing over the last four weeks, rather than how you would like them to feel or behave.

Thank you for taking time to complete this questionnaire. Your feedback is an important part of your child's Mind Crew journey.

Please complete the Questionnaire below to secure your child's place on the Mind Crew Programme.

How your information is used

The information you provide in this questionnaire will help us understand your child's emotional well-being, confidence, resilience and relationships.
